The HIA Health team is dedicated to care for those living with a serious illness, helping individuals and their loved ones live their best. When serious illness creates barriers to leaving home, care is often managed through an emergency room visit or hospitalization.
The HIA Health team includes HIA Hospice for hospice care in the home or in HIA Heather’s House, HIA Primary Care for primary care at home or in our HIA Primary Care Clinic, and HIA Palliative for palliative care in the home. The HIA Health team is focused on quality of life, replacing reactive, crisis-oriented care with proactive, preventive medicine that will reduce unnecessary emergency room visits, hospitalizations and 30-day readmissions while also helping people remain at home.
