Our nurse will make a complimentary visit to meet with your patient and their loved ones, completing a clinical assessment, listening to what is most important to them, and sharing how we will support them. Patients and families benefit most by hospice care when provided for months. Your referral connecting them to our care helps patients and families know the benefits and accept our care when the time is right.
