- Independent, assisted living and memory care’ residents will benefit from HIA Primary Care and HIA Palliative with a physician or nurse practitioner proactively managing their care, whether as their primary care provider or the collaborative palliative care specialist. Their living room is the waiting room where we are managing care.
- As the disease progresses, our HIA Primary Care team seamlessly collaborates with our HIA Palliative and HIA Hospice teams, connecting individuals to the hospice benefit when appropriate, allowing families and loved ones to be supported holistically over the next days, weeks, and months.
- By connecting the right care, at the right time, we support the individual to remain home for as long as possible.
- If a higher level of care is needed, we are part of that process and transition for the individual and their loved ones.
How do we communicate with other providers?
- The HIA Health team closely collaborates with other providers via phone, secure email, secure fax or mail.
- If an individual is receiving care from HIA Primary Care, our physician or nurse practitioner collaborates directly with other specialists to oversee all care needed.
- If the individual is receiving care from HIA Palliative, our physician or nurse practitioner collaborates with the primary care provider and other specialists, providing specialized care focused on symptom management that is delivered in their home.
- If the individual is receiving HIA Hospice care, the medical team collaborates with the person’s attending provider according to their preferences. Most of the time, our Hospice medical director and team directly manage care specific to terminal and related illnesses.
- In all models of care, the HIA Primary Care, HIA Palliative and HIA Hospice teams work directly with your facility staff regarding the care plan, perhaps attending care conferences, supporting your team, and providing an extra layer of care.
Do all residents have to sign up for HIA Primary Care or HIA Palliative to partner with you?
No. We see as many or as few of your residents in your facility as would like to receive care from HIA Primary Care.
Is there a cost to a facility to partner with HIA Primary Care for its residents?
There is no charge or cost to a facility for the convenience of having HIA Primary Care provide care to your residents.
Will we always have the same team of clinicians caring for our residents?
When possible, your facility will have a consistent provider, however your residents may be seen by one of our other clinicians depending on scheduling, availability and other urgent needs. Our providers will all have access to your medical records, for seamless care and support.
How can partnering with HIA Health reduce my operating costs?
HIA Health can reduce the cost of transportation to and from appointments. Your staff will also benefit by having one point-of-contact for coordinating appointments, and ordering medication and testing. HIA Primary Care, HIA Palliative and HIA Hospice can visit multiple residents in a single day, which can also help maximize your staff’s efficiency in daily scheduling and planning.
Is HIA Hospice duplicating services with care in an assisted living or skilled nursing facility?
No. HIA Hospice delivers a set of specific services to the patient and their loved ones. Hospice care is very different from the general medical services and quite specialized. Your staff in the assisted living or skilled nursing facility provides residential, custodial services just as a family would provide in a home setting, while hospice tends to the physical, emotional, and spiritual needs of the patient. The roles of hospice and your team are complementary, supportive, and distinct.
Is hospice paid twice for the care they provide in a facility?
No. Most hospice patients who reside in an assisted living or skilled nursing facility have third-party insurance (Medicaid or private insurance) or they pay out of pocket for their custodial care in the facility. Hospice is paid specifically to provide end-of-life care, which includes medical care, personal care, emotional support, spiritual care, volunteer compansionship and grief support. Additionally, hospice provides medial equipment and medicationis for the terminal disease and related conditions.

The HIA Hospice staff are experts in comfort care. Hospice brings additional expertise to specifically enhance quality of life and comfort. This approach compliments and layers seamlessly with the care provided in your facility.

How do my residents become new HIA Health patients?
